download the form. When you have downloaded it,
please save the form in a 97-2003 Word [Compatibility Mode] format (only
applicable if you have Word 2007). Please save it as: CountMyTrees_your
last name_date submitted (i.e., CountMyTrees_Smith_5.14.2009).
You may have a small window pop up or a message bar across the top of the screen
appear. It should say "Security Warning" or "Security Alert - Macros & ActiveX."
In order to fill out the form, you must select "Enable this Content." The fields
you will type in require this feature to be activated. Please call our office

Fill out the form as directed. Click on the gray boxes when selecting Place of
Purchase or the Tree Type Planted to choose from the options. and then send it
as an attached file by email to
plantamillion@hhrcd.org. Please put "Count My Trees" as the subject line.
